In Indiana, we offer Structured Family Caregiving (SFC)—a supportive solution that allows you or your loved one to receive care from someone you know and trust, right at home.
What is Structured Family Caregiving (SFC)?
Structured Family Caregiving is a service that allows individuals to remain at home while receiving daily support, be cared for by a family member or familiar caregiver, and maintain their independence, privacy, and dignity.
Two Options, One Goal: Supporting You at Home
Option 1: PathWays Waiver
- for individuals aged 60 and older
- Administered by the Division of Disability and Rehabilitative Services (DDRS)
- Both options help families provide care–and receive financial support for doing so
Option 2: Home Health Waiver
- For those 59 and younger
- Administered by Office of Medicaid Policy and Planning (OMPP)
- Both options help families provide care–and receive financial support for doing so
Through these waivers, family members providing care have a source of income.
Structured Family Caregiving Information
What We Do:
If families contact the local ResCare team member, we assist them in contacting the local Area Agency on Aging coordinator so they can determine the level of care. Our team will guide the application process by answering any questions in plain language and providing a checklist of documentation needed.
Who is Eligible?
- Care recipient must be eligible for Indiana Medicaid – need help with at least 3 activities of daily living (ADLs)
- Care recipient must live full-time with their caregiver
- Caregiver must be at least 18 years old and be related (not a spouse)
- Caregiver must pass background checks
Program Benefits:
- Caregiver receives payment
- Ongoing support and training through ResCare
- Professional oversight without institutional care
- Keeps families together
How to Enroll – Step-by-Step:
(1) Contact your local Area Agency on Aging (AAA)
(2) Complete a functional needs assessment
(3) Receive confirmation of waiver eligibility
(4) Choose ResCare as your SFC provider
(5) Begin daily caregiving and receive support
