What is Structured Family Caregiving (SFC)?
Structured Family Caregiving is a service that allows individuals to:
- Stay at home while receiving daily support
- Be cared for by a family member or trained provider
- Maintain independence, privacy, and dignity
Two Options, One Goal: Supporting You at Home
Option 1: Health and Wellness Waiver
- Ages 18-59
- Administered by the Division of Disability and Rehabilitative Services (DDRS)
- Both options help families provide care–and receive financial support for doing so
Option 2: Pathways for Aging Waiver
- Ages 60+
- Administered by Office of Medicaid Policy and Planning (OMPP)
- Both options help families provide care–and receive financial support for doing so
