Through Indiana’s Aged and Disabled (A&D) Waiver, we offer Structured Family Caregiving (SFC)—a supportive solution that allows you or your loved one to receive care from someone you know and trust, right at home.

 

What is Structured Family Caregiving (SFC)?

Structured Family Caregiving is a service that allows individuals to:

  • Stay at home while receiving daily support
  • Be cared for by a family member or trained provider
  • Maintain independence, privacy, and dignity

 

Two Options, One Goal: Supporting You at Home

Option 1: Health and Wellness Waiver

  • Ages 18-59
  • Administered by the Division of Disability and Rehabilitative Services (DDRS)
  • Both options help families provide care–and receive financial support for doing so

 

Option 2: Pathways for Aging Waiver

  • Ages 60+
  • Administered by Office of Medicaid Policy and Planning (OMPP)
  • Both options help families provide care–and receive financial support for doing so
