ResCare Community Living continues to develop integrated, holistic care models that offer best-in-class care, service and medication management to complex populations. We are pleased to share the 2021 clinical outcome highlights from our partnership between ResCare, Ohio and Home-Based Primary Care provider Western Reserve Medical Group.
Provision of Home-based Primary Care to individuals with IDD in Ohio was associated with:
- 44% DECREASE IN HOSPITALIZATION RATE
- 84% DECREASE IN 30-DAY READMISSION RATE
- AVERAGE OF 6 ADDITIONAL DAYS SPENT AT HOME PER YEAR
In 2021, Home-Based Primary Care was made available to ResCare, Ohio clients, with more clients/guardians electing the care model as the year progressed. As the model took effect, we saw very significant improvements in clinical/utilization outcomes – some of the largest effects seen in over 15 years. ResCare clients with IDD spent an average of 360 days per year at home in the community, outside of hospitals and nursing homes.
